Community Vaccine Survey
Hello, my name is Dr. George Gellert, and it is my pleasure to serve as the Deputy Health Officer for the County of San Benito. On behalf of Public Health Services, I am asking you to complete a 3–5-minute survey that will help us understand and better address the concerns county residents and workers may have about getting the COVID-19 vaccine.
Your feedback is extremely important in helping us improve our vaccine delivery. All information collected is completely anonymous and confidential, and no individual survey responses will be analyzed or shared - only all survey responses together in aggregate.
