Deputies arrested Jack Julfayan of Glendale for forging a narcotics prescription and alter/forge identification

SUSPECT:      Jack Julfayan, 38-years-old of Glendale
 
Summary:
On February 25, 2021, deputies from the Rancho Cucamonga Station responded to a pharmacy regarding several forged prescriptions.  Management at the pharmacy reported that on January 26, 2021 and January 27, 2021, a male doctor called in 26 prescriptions for four different patients.  All the prescriptions were picked up by a male subject with an Arizona driver’s license who claimed to be the caretaker of the four patients.  When a request was made for a refill a month later, the pharmacy contacted the doctor for authorization.  The doctor, who is a female, immediately recognized the prescriptions as fraudulent and denied the requests. 
 
On May 12, 2021, after a lengthy investigation, deputies identified the suspect as Jack Julfayan.  Deputies contacted Julfayan at his residence as he was parking his vehicle and detained him.  A vehicle search was conducted and deputies located an altered Arizona driver’s license, 15 Vicodin pills, and a blank prescription slip belonging to a doctor in Los Angeles. 
 
Inside the home, deputies located more than 25 prescription bottles that were not prescribed to Julfayan, a prescription pad belonging to the doctor in Los Angeles, several documents with various names on them, and a stolen firearm that was fully loaded. 
 
A records check revealed Julfayan had an outstanding warrant out of Los Angeles for forging a narcotics prescription. 
 
Julfayan was arrested and booked in at the West Valley Detention Center where he is being held on $100,000 bail and an additional $200,000 bail for the Los Angeles County warrant.
