The AFD CORE Team's Mission to Improve Community Health and Wellness

In the spring of 2018, the AFD undertook an analysis of its EMS responses and hospital transports to identify and evaluate methods to improve system efficiencies. The results provided evidence that the Department’s EMS deployment model was strained as it’s designed for time-sensitive, life threatening emergencies, and not built for significant numbers of low acuity and non-emergent medical responses.

In response, a community needs assessment was completed and the AFD established the Community Outreach Referral and Education (CORE) Team. As part of the Department’s community outreach efforts and under the direction of Assistant Chief Erich Scheunemann, one of the primary objectives of the AFD CORE Team is to improve the health and wellness of high utilizers of 911 by integrating EMS with partners in community healthcare and social services.

By providing patient-centered services in the out-of-hospital environment, the mobile healthcare component of the AFD CORE Team is currently staffed by a single Firefighter/Paramedic, who directly responds to clients and provides alternate solutions for their healthcare needs and navigates them to other more appropriate resources than a hospital’s Emergency Department.

With the mission to provide the right care, at the right time, in the right setting, with the right resources, and at the right cost the AFD CORE Team is currently seeking means by which to expand its services to the community. This will include an increase in capacity and overall efficacy of services provided by moving to 24 hour/7 days-a-week availability and will include additional Firefighter/Paramedics as well as behavioral health specialists and social workers.
