The Bloomfield Department of Health and Human Services is conducting a Community Health Assessment survey!

Your input is key and will remain anonymous. The survey will help give a complete health picture of Bloomfield so we may provide the proper health programming, access and education to all residents.

You may pick up a hard copy of the survey at the Health Department or take it online at the link below. Hard copies of the survey may be returned to the Survey Drop-Box at the Health Department in Rm. 111, 1 Municipal Plaza, Bloomfield, NJ 07003.
