The Chula Vista Police Department is requesting assistance in locating missing an adult male at risk by the name of Anthony Davis.

Mr. Davis was last seen at his board and care facility at 1245 Finch Place in Chula Vista on 02-14-2014 at approximately 4:30 pm. He is known to wander away but usually returns in a timely manner. Mr. Davis suffers from schizophrenia and mild dementia. He was last seen on foot wearing black and yellow new shoes, black and gray sweatpants and a gray sweatshirt. He may also be wearing a white medical bracelet.

Mr. Davis is 55 years old, 5'7" tall, 150 pounds, blue eyes, with gray balding hair and a gray beard.
