SUMMARY: On Saturday, September 20, 2025, at approximately 11:36 p.m., deputies from the Apple Valley Sheriff’s Station, rescue personnel from the Apple Valley Fire Department, and American Medical Response responded to a two-vehicle traffic crash that occurred on Kiowa Road near Kiowa Place, in the town of Apple Valley.

Through investigation, deputies determined a white Nissan Altima, driven by Exon Herrera Hernandez, drove northbound on Kiowa Road approaching Kiowa Place behind a white Hummer H2, occupied by a 44-year-old male (driver) and Salvador Rubio Tamayo (passenger). Herrera Hernandez rear-ended the Hummer, causing it to leave the roadway and roll. The driver and Rubio Tamayo of the Hummer suffered major injuries and were transported to hospitals by ambulance. The driver of the Hummer later succumbed to his injuries and was pronounced deceased at the hospital. Rubio Tamayo remains hospitalized in stable condition.

Through investigation, Herrera Hernandez was found to be under the influence of alcohol. Herrera Hernandez was arrested for Penal Code 191.5(a), vehicular manslaughter while intoxicated with gross negligence, and was booked at High Desert Detention Center in Adelanto.

An investigator from the San Bernardino County Sheriff’s Department Major Accident Investigation Team (M.A.I.T) arrived and is conducting an investigation. Both lanes of Kiowa Road were closed between the intersections of Del Oro Road and Caribou Avenue for approximately seven hours, while deputies processed the scene for evidence and interviewed witnesses.
