SUBJECT LOCATED (Missing elderly man with dementia. Assistance needed in locating.) 
CHANG WAS LOCATED AND RETURNED TO THE CARE FACILITY.
<p>On 10/26/16 around 0530 hours, Yang Chang walked away from his care facility in Walnut Creek. Chang has memory issues but is otherwise healthy and a history of walking away from care homes in the last year. He was relocated from a care facility in Oakland to the Walnut Creek location less than a week ago so he may try to travel back to the Oakland area. He was found walking around Oakland and Berkeley in the previous incidents.</p>

<p>Chang is a 71 year old Asian male. He is 5’8” tall and weighs 155lbs with grey hair and brown eyes. He was last seen wearing a white shirt and khaki pants.</p>
