Green Bay Police have requested that a silver alert be issued for a missing Green Bay man. Philip Jeanquart, age 68, suffers from dementia. He last had contact with family members at approximately 12:00pm on 03-10-15. Philip may be driving a 2008 Buick Lucerne, gray in color, WI license plate 854-WMN. Philip also has the family pet with him, a gray and white Shih Tzu. The public is asked to stay alert and contact the Green Bay Police Department with any information at 920-448-3200.

Silver Alerts are issued for missing individuals believed to be suffering from dementia or some other cognitive impairment.
