Last seen wearing Light blue t-shirt, beige khaki shorts. Last seen: Sunrise West Condos, Palm Springs at noon 01-15-15. Jack is riding a yellow mountain bike with a purple helmet. He suffers from short term memory loss and possibly showing symptoms of Alzheimer's.
