Sex offender release notification
The Green Bay Police Department is releasing the following information on a convicted sex offender who is being released into our community on November 11th 2014. This release of information will enhance public safety and raise community awareness.
Chester J Kusiak, a 55 year old male / white was convicted of 1st Degree Sexual Assault of a Child in 2001 and 2003. The condition of his release are as follows:
1. No unsupervised contact with minors; No taverns/bars/liquor stores; No contact with victim(s)
2. Not to consume alcohol or drugs
3. Comply with standard sex offender rules; Cooperate with electronic monitoring.
4. Face-to-Face contact with Law Enforcement required; Comply with all requirements and lifetime registrant of WI Sex Offender Registration Program.
5. Lifetime GPS
Mr Kusiak will be released on November 11th 2014 and will live at the Transition Living Place (TLP), 1761 Shawano Ave. He will remain on supervision until August 14th 2027.
