A Vital Resource For Quality Health Care

The Public Health Department Health Care Centers celebrate National Health Center Week, August 10 – 16, 2014. Health Care Centers provide local solutions for affordable and accessible health care.

Throughout Health Center Week, our Health Care Centers will hold receptions open to the community to learn more about our services. They will have information, food and giveaways to share at the following locations and times.

Monday, August 11, 2:00 – 4:00 pm
Lompoc Health Care Center, 301 North R Street

Tuesday, August 12, 12:00 Noon – 1:00 pm
Santa Maria Health Care Center, 2115 S. Centerpointe Pkway

Wednesday, August 13, 12:00 Noon – 1:00 pm
Franklin Health Care Center, 1136 E. Montecito Street

Thursday, August 14, 12:00 Noon – 1:00 pm
Carpinteria Health Care Center, 931 Walnut Avenue

Nationwide, Community Health Care Centers have been in existence for more than 45 years, serving more than 22 million people. Whether they are uninsured or recipients of Medi-Cal and Medicare, many residents choose Health Care Centers because they are convenient, affordable and offer a range of services from a team of caring professionals. Residents who receive preventive care and office based services from Health Care Centers, experience reduced hospitalizations and unnecessary visits to the Emergency Room.

Our goal is to provide the highest level of quality care in each of our Health Care Centers located throughout the County, from Santa Maria to Carpinteria. PHD has initiated many recent activities including:

• Re-designing systems to streamline the reception and appointment processes for our patients.

• Certifying as a Patient Centered Medical Home which means high levels of coordination and continuity of care for patients.

• Enrolling people into Medi-Cal at any of our Health Care Centers. For those interested in enrolling into Covered California, we will begin during the open Enrollment period in November.

• Enhancing our electronic health record (EHR) to provide timely and accurate information and enabling patients to access their patient records through our internet-based patient portal.
